Pueblo County and Other Participating Counties/COGS USDA Commodities Supplemental Food Program (CSFP)

Eligible Participants:

• Women who are Pregnant, Postpartum, or Breast Feeding must provide doctor’s documentation (not participating in the WIC Program)
• Children ages 1 thru 6 yrs. (not participating in WIC program) needs to bring birth certificate and shot records
• Elderly (60 years of age or older)

Monthly Income Guidlines (2008):

ALL APPLICATIONS MUST BE COMPLETED IN PERSON BY THE INDIVIDUAL APPLYING FOR SERVICES

To complete an application please bring the following items:

• Colorado Picture ID
• Proof of Total Income of Household
